Here are some points about our country’s approach to fighting the C19 coronavirus.

The main reason the media reporters and the administration are talking past each other about what C19 response policies to follow or even how to evaluate what’s being done is that no one is astute enough to put their finger on the widely different and competing utilities or figures of merit that the various parties are trying to maximize.  Here are three distinct ones.

C19utilities

UTrump tries to find a balance between the economy (GDP$) and the mortality of C19 (total number dead) by appropriately trading off weighting constants C1 and C2.  I’m not sure that any of them on Team Trump are either bright enough or courageous enough to tell the President that this is what his gut is telling him to do, and that this can be put on a more rigorous basis for making defensible policy decisions.

When we listen to medicos like Drs Fauci or Brix, they claim to be apolitical, and the policies they recommend attempt to maximize UMedico by minimizing the number of dead.  None of those guys concern themselves with the economy or the longer term health impacts of an economically destitute country.  They focus on the here and now of saving lives, period.

And then there’s the Democrats whose utility is all about getting back into power and putting the country on a solid road to socialism cum communism.  They could give a big rat’s ass about a recovering economy or minimizing the number of C19 deaths.  Their utility UDems is the converse of UTrump.  The want the economy to take the biggest conceivable hit, and have as many people die as possible, all of which, no matter the numbers, they will blame on Team Trump.  Their Big Lie will be that things would have been much better under a Dem (Hillary?) administration, and we’ll be happy dancing again as soon as we throw Trump out of office.

If those reporters would understand any of this, they could perhaps quit asking profoundly stupid questions and write some cogent reports that sort out the apparent differences between the gaggle of factions in the public forum.

C19poster01Then there’s the matter of hydroxychloroquine (HCQ) and azithromycin (AZ) the policies toward which I have covered elsewhere in these pages.  Trump and I promote the use of these proven malaria fighting and anti-biotic drugs for ameliorating C19 both as a prophylactic and as a therapeutic.  The tests (albeit of limited sample sizes) that have been done with these drugs in treating C19 have already delivered impressive results.  And a big study is underway to measure its effectiveness.  But I (and Trump?) take great hope in promoting their immediate use in that they are already being successfully used in the US in this pandemic, and healthcare workers, led by the physicians, are prescribing them for their own use and hoarding the supplies of HCQ and AZ whenever they can get their hands on it.  They know these drugs are safe, and they have nothing to lose in taking them.

So here’s the Rebane Doctrine for the prescription and use of HCQ and AZ.  The Dems are on record vilifying the President for being a proponent of something they consider to be unproven, ineffective, and possibly dangerous.  Whether they believe their own crap is irrelevant, refer again to the utility function discussion above.  Almost everyone on the Right wants HCQ and AZ as quickly as they can get their hands on it.  And if we believe the numbnuts on the Left, they want nothing to do with it, and would be glad to have the folks on the Right scarf it up.  So as these two drugs become available, let’s have the Repubs and the Dems agree on a distribution policy based on party registration.  Those who are registered Repubs are first in line to get the drugs, and those registered as Dems either don’t get them, or are last in line if they have a belated epiphany.  Both sides win; what’s not to like?

[6apr20 update]  And here is some more dismal news on how government bureaucracies are impeding the fight against C19 in their approach to approving and/or promoting the use of HCQ and AZ by hospitals and our personal physicians. These are the same guys to whose care and feeding our Left wants to turn over our nation’s healthcare industry.  And here more evidence from the field comes in.  (H/T to readers)

[10may20 update] HCQ as a SARS prophylaxis was discovered over 15 years ago, and (to my knowledge) first published in the Virology Journal here.  HCQ’s successful clinical uses against C19 have been reported from both Wuhan and Singapore, and most recently its robust use in Costa Rica (here).  Further reports about its successful uses, especially preventing progress of the disease in early onset patients, is becoming available in dribs and drabs from all over, including the US.  Why ‘dribs and drabs’?  That is because here in America the Left has politicized the attempts at its widespread clinical uses for the obvious reasons involving President Trump. (more here)  We recall, that ALL things new in science and technology have had to surmount intense opposition from the then entrenched establishments, both technical and political.  In this litigious land it takes rare courage for physicians to wander too far from the CYA comforts of ‘standard of care’, and the Left and their lamestream lackeys have driven a stake in the ground about HCQ being the one therapeutic that is too far afield – ‘Physician, be forewarned!’

But rejoice, the pushback on the Left has now progressed from ‘HCQ is ineffective, harmful, and dangerous’ to a ‘Large Study Suggests Malaria Drug Hydroxychloroquine Doesn’t Help COVID-19’ in the just published 7may20 New England Journal of Medicine.  Therein the strongest statements against HCQ that the authors can make is that (all italicized emphases mine)

  • “Hydroxychloroquine has been widely administered to patients with COVID-19 without robust evidence supporting its use.”
  • “The study found no significant link between use of hydroxychloroquine and intubation or death.”
  • “Our strong feeling is the drug should not be routinely administered to patients with COVID-19.”

What they all avoid denying is the effectiveness of HCQ in the early onset phases of C19.

As more evidence of the ‘dribs and drabs’ that substantiate HCQ uses, consider two recent French studies – here and here.

The bottom line is that no medicine is 100% effective for all cases of any single disease in all of its phases.  But HCQ is now known to be sufficiently effective in the treatment of C19, enough to scare the bejeezus out of Left’s leadership as they attempt to fashion their anti-Trump campaign, while saddled with their champion Bumblebrain sporting Pelosi’s kerchief in his gauntlet.

    A very common punch thrown at trump is that he said/did X when expert Y said to do Z. The palooka who uses that tactic is generally ignorant of opinions contrary to Y’s or just does not care; the important thing is to follow the Expert/High Priest imbued with the authority to declare it halal or haram.
    Everyone points to the FDA’s role in keeping Thalidomide out of the US (mostly a good thing) but most don’t know that regulator said no to just about everything that came across her desk.
    It was spooky to purchase Thalidomide in Grass Valley in 2001.

    Another Doctor Reports Dramatic Improvement In COVID-19 Patients Using Trump-Touted Treatment


    As pro-establishment mouthpieces downplay the efficacy of hydroxychloroquine to treat COVID-19 as “anecdotal” with “little evidence that the treatment is effective,” yet another doctor treating has claimed dramatic improvement in coronavirus patients within hours of taking the anti-malaria drug in combination with two other medications.
    Los Angeles doctor Anthony Cardillo says he’s seen very promising results when the Trump-touted drug is combined with zinc for severely-ill coronavirus patients.
    “Every patient I’ve prescribed it to has been very, very ill and within 8 to 12 hours, they were basically symptom-free,” Cardillo told Eyewitness News, adding “So clinically I am seeing a resolution.”
    Cardillo, CEO of Mend Urgent Care, says that the drug must be used in conjunction with Zinc, as the hdroxycholoroquine opens a ‘channel’ for the mineral to enter cells and prevent the virus from replicating.

    No fish, for my first wife’s end stage cancer once her ability to tolerate standard chemotherapies ended. Her last month.
    The horrific birth defects was because it interferes with the ability to branch off blood vessels, bad if it’s to be a limb of an embryo, good if it’s a tumor growing its blood supply.

It seems, therefore, that the paper was published at that time partly to help justify a change in the messaging. A “U-turn” doesn’t seem like quite the right term, therefore, for what happened. “

    I have a theory on that. Locally, they appear to be running rather wild what with the lack of active policing of the mildly wicked.
    My guess is that they don’t handle each other’s stuff all that much, perhaps glass pipes excepted, as much as the straights. No keyboards, door knobs, handrails and the like. Plus, and maybe more importantly, they aren’t in enclosed spaces as much.
    You really do have to wonder what the point of making the homeless shelter up was. Any kind of basketball-gym-with-cots is where the real viral action is going to happen.
    I did see that everybody was kicked off the beach in Venice Beach except for the Blue Mob’s favorite folks though.

    Honestly, it would be nice to have a Famous Economist Guy who wasn’t a political activist. There’s so many knobs on their ‘science’ calculations that bias would make you an impractical practitioner. Maybe witch doctors were always that way.
    In any case, I’ve been inundated for years with theories concerning vaccines, 5G, blue green algae, vitamin C megadoses, life saving fermentation of food, microcurrents, blah blah, and they never come out of the speaking orifices of right wingers. It’s possible that it’s bipartisan, but given the heavy duty orientation of such beliefs in single older cat ladies (who tend to not be pro-GEOTUS), I find it hard to believe.

    I had a post a day or two ago (which, naturally I can’t find due to typepad’s crummy search) wondering whether all the fuss about ventilators wasn’t misdirected. They’re not that useful and it would be interesting if they weren’t the right answer in any case. There seemed to be increasing favor rumbling around out there for CPAP and it’s brethren. At this point, like off-label drugs, ventilators are as much a political football as anything.

    Oh well, anything to win an election.
    It is pretty wild to view the different interests in the result, to riff on GeorgeR’s equations.
    Trump wants to be re-elected and gains nothing from doing harm. He doesn’t need large business interests to like him, just the voters. Oddly, you end up with a reasonable compromise between destroying/resurrecting the economy and killing grandma if you give a shot at keeping the masses in good humor. Read his tweets accordingly.
    The medical biz is all about health care, surprisingly. Given their druthers, we’d all stay indoors forever, in addition to outlawing guns, cars, alcohol, and the overweight. Read their missives accordingly.
    The Democrats want to win the 2020 election at all costs. Preferred answer is a lot of dead people and a crashed economy followed by a miraculous recovery under Joey Fingers. Read their missives accordingly.
    Everybody has a finger in this pie, even Sen. Biden. Their interests drive their theorizing, just watch your six when the Democratic candidate is in the room.

    We are doing 248,000 tests per day.
    Above the posters here last month were all fans of hydroxychloroquine (HCQ) and azithromycin (AZ)as therapeutics; instead we are increasingly learning that not only are these two drugs largely ineffective, they may be dangerous for those with Covid-19. Yet the President of the United States even today continues to pump them like some 19th century patent huckster.

    Finally when we talked on April 6 there were 10,000 dead; today less than five weeks later there are 80,000 deaths and the rate indicates that we will have 175,000 dead by the end of June.

    Right now, the USA has administered about 9.4 million tests, and had a total of 1.4 million cases confirmed. Back of the envelope calculation, assuming three tests per confirmation (the test establishing the patient status, and two confirming recovery).
    With 330 million people in the USA… how many tests do the fans of testing as the key to beating the virus expect to administer?
    The next most test crazy country is Russia at 5.4million, then comes another nine in the low seven figures.
    This isn’t syphilis, Steve. It’s hard to do tracking down of who infected who… as in Tom Lehrer’s joyful “I Got It From Agnes”… outside of a police state.

    It is so bloody obvious to everyone not afflicted with TDS that it’s an anti-Trump action, not anti-HCQ… as everyone knows it will take a year or more for randomized double blind studies to take place and, as the French virologist from Marseilles… the game plan is delay everything that might be a positive for Trump until after the election.
    All I want going forward is some treatment that improves my odds if I get the SARS du jour, from Wuhan. I’d be happy if someone could take the Wayback Machine and convince Trump to shut up about it until about July but even if the Wayback Machine could be found and Mr. Peabody is amenable, Trump might not play ball with Sherman no matter how hard he whines.

    Simple Gregory, we need enough tests and capacity to trace to put 33 million of your fellow Americans back to work, which the nations Governors, who the President says are in charge of making this decision, estimate at approximately 3 million tests per day for the foreseeable future.
    If Trump had scaled our capacity to test in late February we would be there today. He did not, so people won’t go back to work, if some people do go back to work many people won’t go to stores, restaurants and events, and the economy will remain in the crapper.
    We are the greatest country in the world, but if our leader is a narcissistiic, dissembling, indecisive, man boy all that American innovation and capacity can’t buy its way out of our current depression.

    We only need the tests that are required to provide a diagnosis for new cases of suspected infection. We got that now.
    Right now, between the new facility at the GV Veterans Memorial building, and somewhere in Truckee, there’s assembly line testing that can handle 135 a day. It’s unclear how many are showing up to be tested as there ain’t been an update in the number of tests since Monday.

    Checking The Union story of the Veterans Memorial test location… they were promising 135 tests a day. Let’s call the capacity between Truckee and GV to be 250 a day. It’s been eight days so… tomorrow, there should be 3,469 total tests, more than double the current total.

    The Dem proposals I have seen require a force of 100,000 testers going out testing millions. That alone would take a couple months to launch. Besides, with all the bad tests out there we have been relying on, we are just starting to get pur hands on tests with 90% accuracy as they are being developed. One in ten bad. Before that and recently, the tests were 70% accurate, before that when the Chinese Virus was unleashed on the innocent across the globe, some tests were only 50% reliable.
    I think it is best to wait for the new test that is 99% accurate and tests for antibodies as well…according to the manufactures boast. Anti-bodies is where its at. Under 5 years old? Got a 0.0014 chance of dying from the Kung Flu. Under 50 and healthy? 0.02 chance. As mentioned before in one study, 7 out of 8 adults who tested positive had no idea that had it or showed any symptons.
    Antibody testing is where it is at. Since I have not been tested, i just assume I have it and act according for others sake. Or, I assume others have the Plague and I act accordingly.
    What would Biden do differently? Stupid question. Took him til April 4th to decide that travel restrictions with China and Europe was a good idea, and necessary. So, its up to Trump to get ‘er done cause nobody else can or will. Thats a good thing. Government bureaus are like turning an aircraft carrier around. They ain’t nimble or agile.
