Here are some points about our country’s approach to fighting the C19 coronavirus.

The main reason the media reporters and the administration are talking past each other about what C19 response policies to follow or even how to evaluate what’s being done is that no one is astute enough to put their finger on the widely different and competing utilities or figures of merit that the various parties are trying to maximize.  Here are three distinct ones.

C19utilities

UTrump tries to find a balance between the economy (GDP$) and the mortality of C19 (total number dead) by appropriately trading off weighting constants C1 and C2.  I’m not sure that any of them on Team Trump are either bright enough or courageous enough to tell the President that this is what his gut is telling him to do, and that this can be put on a more rigorous basis for making defensible policy decisions.

When we listen to medicos like Drs Fauci or Brix, they claim to be apolitical, and the policies they recommend attempt to maximize UMedico by minimizing the number of dead.  None of those guys concern themselves with the economy or the longer term health impacts of an economically destitute country.  They focus on the here and now of saving lives, period.

And then there’s the Democrats whose utility is all about getting back into power and putting the country on a solid road to socialism cum communism.  They could give a big rat’s ass about a recovering economy or minimizing the number of C19 deaths.  Their utility UDems is the converse of UTrump.  The want the economy to take the biggest conceivable hit, and have as many people die as possible, all of which, no matter the numbers, they will blame on Team Trump.  Their Big Lie will be that things would have been much better under a Dem (Hillary?) administration, and we’ll be happy dancing again as soon as we throw Trump out of office.

If those reporters would understand any of this, they could perhaps quit asking profoundly stupid questions and write some cogent reports that sort out the apparent differences between the gaggle of factions in the public forum.

C19poster01Then there’s the matter of hydroxychloroquine (HCQ) and azithromycin (AZ) the policies toward which I have covered elsewhere in these pages.  Trump and I promote the use of these proven malaria fighting and anti-biotic drugs for ameliorating C19 both as a prophylactic and as a therapeutic.  The tests (albeit of limited sample sizes) that have been done with these drugs in treating C19 have already delivered impressive results.  And a big study is underway to measure its effectiveness.  But I (and Trump?) take great hope in promoting their immediate use in that they are already being successfully used in the US in this pandemic, and healthcare workers, led by the physicians, are prescribing them for their own use and hoarding the supplies of HCQ and AZ whenever they can get their hands on it.  They know these drugs are safe, and they have nothing to lose in taking them.

So here’s the Rebane Doctrine for the prescription and use of HCQ and AZ.  The Dems are on record vilifying the President for being a proponent of something they consider to be unproven, ineffective, and possibly dangerous.  Whether they believe their own crap is irrelevant, refer again to the utility function discussion above.  Almost everyone on the Right wants HCQ and AZ as quickly as they can get their hands on it.  And if we believe the numbnuts on the Left, they want nothing to do with it, and would be glad to have the folks on the Right scarf it up.  So as these two drugs become available, let’s have the Repubs and the Dems agree on a distribution policy based on party registration.  Those who are registered Repubs are first in line to get the drugs, and those registered as Dems either don’t get them, or are last in line if they have a belated epiphany.  Both sides win; what’s not to like?

[6apr20 update]  And here is some more dismal news on how government bureaucracies are impeding the fight against C19 in their approach to approving and/or promoting the use of HCQ and AZ by hospitals and our personal physicians. These are the same guys to whose care and feeding our Left wants to turn over our nation’s healthcare industry.  And here more evidence from the field comes in.  (H/T to readers)

[10may20 update] HCQ as a SARS prophylaxis was discovered over 15 years ago, and (to my knowledge) first published in the Virology Journal here.  HCQ’s successful clinical uses against C19 have been reported from both Wuhan and Singapore, and most recently its robust use in Costa Rica (here).  Further reports about its successful uses, especially preventing progress of the disease in early onset patients, is becoming available in dribs and drabs from all over, including the US.  Why ‘dribs and drabs’?  That is because here in America the Left has politicized the attempts at its widespread clinical uses for the obvious reasons involving President Trump. (more here)  We recall, that ALL things new in science and technology have had to surmount intense opposition from the then entrenched establishments, both technical and political.  In this litigious land it takes rare courage for physicians to wander too far from the CYA comforts of ‘standard of care’, and the Left and their lamestream lackeys have driven a stake in the ground about HCQ being the one therapeutic that is too far afield – ‘Physician, be forewarned!’

But rejoice, the pushback on the Left has now progressed from ‘HCQ is ineffective, harmful, and dangerous’ to a ‘Large Study Suggests Malaria Drug Hydroxychloroquine Doesn’t Help COVID-19’ in the just published 7may20 New England Journal of Medicine.  Therein the strongest statements against HCQ that the authors can make is that (all italicized emphases mine)

  • “Hydroxychloroquine has been widely administered to patients with COVID-19 without robust evidence supporting its use.”
  • “The study found no significant link between use of hydroxychloroquine and intubation or death.”
  • “Our strong feeling is the drug should not be routinely administered to patients with COVID-19.”

What they all avoid denying is the effectiveness of HCQ in the early onset phases of C19.

As more evidence of the ‘dribs and drabs’ that substantiate HCQ uses, consider two recent French studies – here and here.

The bottom line is that no medicine is 100% effective for all cases of any single disease in all of its phases.  But HCQ is now known to be sufficiently effective in the treatment of C19, enough to scare the bejeezus out of Left’s leadership as they attempt to fashion their anti-Trump campaign, while saddled with their champion Bumblebrain sporting Pelosi’s kerchief in his gauntlet.

    BillT 558pm – An important error in the piece right from the gitgo – “Masks may(sic) be more important than testing to control COVID-19’s spread.” It should read, ‘Masks are more important than testing to control COVID-19’s spread.’ As we know, testing is snapshot in time, and no one is doing comprehensive and timely testing of any population cohort to even get a snapshot of the C19 progress in that cohort. Small scale testing is used primarily to allocate healthcare resources to those few who have taken the test. And given the pace of C19 progress, the disease itself will quickly tell you whether you need healthcare or not – that’s why they don’t test you and tell you to go back home even when you show up with mild symptoms. It could be different if there were copious amounts of HCQ and AZ available.

    George – comment on the docs prescribing for their own:
    Again, from my daughter, a PharmD – She received letters from the Pharmacy Boards of Nevada and Iowa, (she’s licensed in both states) in the last week of March, stating that it was illegal for pharmacists to fill Rx for the chloroquines unless it was an existing customer (RA or Lupus); was for less than 14 days; or for a trial. The reason- so many docs were prescribing it for their relatives that quantities were being hoarded, and the ones who needed those meds might be in danger of not having them available.

    Naturally the response to my comment last week was precisely what I expected…more Trump ass kissing from the congregation….
    Just wondering though, I am all for masks, but if testing helps us understand the patterns of exposure and spread, and intervene earlier with therapeutics, then we should be for testing as well, right? Amongst the nations most effective in slowing the spread have been several that have instituted widespread testing, including Australia, Singapore, South Korea, Germany and others.
    So after President Trump promised on March 6th, a month ago, that, “Anyone who wants to get tested can get tested,”
    How did your tests go? I am sure all of you are happily isolated secure in the knowledge that you have not been exposed yet.
    “Trump: “The testing has been amazing, actually. What they’ve been able to produce in such a short period of time. You had mentioned 4 million tests before — 4 million? …”
    “Anybody that wants a test can get a test. That’s what the bottom line is. … Anybody right now and yesterday — anybody that needs a test gets a test. We — they’re there. They have the tests. And the tests are beautiful. Anybody that needs a test gets a test.”
    A month ago the President also said that we would be doing 1,000,000 tests per day in less than a week. The next week we were doing 12,00 tests per day and today we are doing slightly less than 100,000 test per day.
    So what was the result of your tests my friends?

    I wouldn’t be quick to label the nv or ia pharmacy boards as politically motivated; if I were to find myself a positive for Wuhan SARS, I’d be happy as a clam with scrip for 13 days of a Chloroquine and a Z-pak.
    A local pharmacist is having z-pak patients take the first two tabs (of 6) on site, noting the time and then taking a tab at that exact time the next four days, and I suspect that was motivated by a run on AZ.

    I wonder how many times a day he thinks about Trump. To me, the ponderous actions of government dealing with an emergency and simply musing about the social effects are way more interesting but, you know, what about Trump?
    Generally, it seems like there’s this notion that the sitting President sits at a giant control panel and makes things happen, at least when he’s not spending a large part of the day fending off baseless impeachment attempts. If you could just get him to press the ‘manufacture a million ventilators’ button.

    It occurred to me that his background, so far as I understand it, might be part of the problem.
    It would surprise the bejeepers out of me if the man has worked in any large organization, especially in a position of responsibility (ironic for a ‘business’ expert, but there you have it). Unless you’ve done that, you really can’t appreciate the rather loose connection between CEOs (/Presidents) and action on the ground. Not to mention the latency.
    His screeds remind me of precocious 22 year olds arguing politics in a bar after an afternoon at Econ 102a. There’s hardly anything worse in the world than a table-pounding young socialist (or libertarian), and some people never grow out of it.

    I doubt that the average person, worried about their job and the supply of toilet paper, really even follows the noises that come out of Pelosi’s Piehole. That’s mostly left to local letters-to-the-editor writers. But I really did like this line:

    That pretty much sums it up. I can’t see that anyone could argue with it regardless of political persuasion.

    How to fight COVID-19? Easy. Well, simple but not always easy. Take all the Dems, Progressives, those with TDS, and the Green Libertarians and march their asses off to the interment camps we have prepared for them….perhaps on some island out in the middle of the deep blue sea or in underground caverns in Utah.     Can you provide some documentation about these tests ? A couple of links will help.

    Yeah George…….just a couple of links……just a couple. Oh sure with the same number of keystrokes Punch could Google them himself but that wouldn’t be his way. Always better to get someone else to provide them and then complain incessantly about that which was provided……not good enough….not exactly what Punch was looking for…..didn’t exactly make the point Punch was trying to make…etc.

    PaulE 846pm – Can’t believe you asked that question Paul. I have provided links every time I have mentioned HCQ and AZ, including in the current post. And since you’re not keeping up with any of the C19 pandemic saga, there is a big study about HCQ/AZ that is being released soon that is contributing to the “light at the end of the tunnel” that the various pharma initiatives now under way will end this pandemic sooner than later. The market responded to this news today. How come you have no idea all of this is going on?
    PS. also give Google a try on this; you know, the big search engine outfit.

    One thing about the off-label use of these drugs, is that we’ll find out soon enough whether they work for some or all patients. They are sure giving out a lot of them. With a 2/3 chance of dying if you’re on a ventilator (which somewhat decreases the real value of the things), a lot of folks are taking the chance.

    This has nothing to do with Paul’s comments.
    It is no secret that the Left’s knee jerk reaction to HCQ/AZ is to nip it in the bud, to dismiss it out of hand, to shut it down now. Must not go there! Must not look that way. Nothing to see here folks, move on. Trump has blood on his hands…..The Boston Globe blared out “Trump has blood on his hands”, but the editorial/news was concerning pulling out of the Paris Climate Accord. 🙂
    It is like the Left is working overtime to squash any hope. Hope is a mainstay. The common denominator of those who attempted suicide and failed is they lost hope….according to interviews of those who survived their suicide attempts. “Without hope the people perish” the ancients wrote.
    It is not like the Left peddles in despair, crisis, doomsday scenarios….it is they peddle nothing but despair, hopelessness, and impending doom currently. Imagine telling an AIDS or cancer patient that there may be a new cure and having those words derided by the Lamestream media at the speed of light.
    Tranquility and the Lamestream Media Arm of the Left cannot co-exist. And never the twain shall meet. Crisis junkies, drama queens, peddlers of chaos, tension, turmoil, and upheaval one and all. Blood sells.
    Last night I read a MD/researcher’s findings on his limited samples of patients who were given HCQ. He said it is only effective if the patient is also given zinc. Those not given zinc with HCQ had no noticeable benefit.
    When Trump said he ‘would love to have the country back to work by Easter…I would love to see that’, he was peddling hope. If a new (old) drug shows promise in small sampling and antidotal results…..well, because it came out of Trump’s mouth, it must NOT be listened to and immediately extinguished. Don’t listen to Trump’s daily COVID-19 press conferences. You must not go there! Listen to us, not Trump. You know us….we are the ones that peddled the Russian Collusion Delusion and we have 12 years before the Climate Crisis dooms us all.

    Hey George– did you know that all three of the trump family trusts have investments in a mutual fund whose main investment is in a company named Sanofi, a French drugmaker that makes Plaquenil, the brand-name version of hydroxychloroquine? The mutual fund, run by major Republican donor Ken Fisher, is the single largest investor in Sanofi. What a coincidence!
    No wonder he keeps promoting the drug contradicting health experts (of which trump nor you are) who say NOT to use the drug without further testing and approval.

    According to 2019 financial disclosure – Trump owns between $1,000 and $15,000 worth of Dodger & Cox mutual fund.
    According to the prospectus, 2.9% of the fund is made up of Sanofi stock.
    Doing basic math, this means that Trump owns anywhere between $29 and $435 worth of Sanofi stock.
    And… Sanofi doesn’t sell Plaquenil in the USA.
    Rich people invest in big pharma is the news flash you’re selling, but this is an old drug whose patent expired during what, maybe the LBJ Administration.

Because the health experts want a couple years of double blind clinical trails covering thousands of patients and multiple tests o make sure the of side effects, and it is totally safe before approving the drug for general population use. Health experts want all risks removed and everyone is healed and nobody dies.
    Tump has been fast tracking various drugs under the Right to Try policy…before the current outbreak. Hey, cancer and AIDS patients were driving to Mexico or flying off to a foreign destinations to receive treatments that were hung up by the FDA. As Trump says, “What do they have to loose.” Right to Try.
    Clinical tests have begun, both private sector and the public sector is working day and night working on a vaccine, cure, or anything that may help. Even if it only helps 10%, its better than having a handful of nothing. And yes, the private sector includes BIG PHARMA, who is bringing top lab boys out of retirement to find a fix.
    Full Disclosure: I have owned Dodge and Cox Mutual fund(s) in the past. I also know that has been common knowledge for quite some time that investing in the health care sector is a good bet. Bio-tech is a little more risky. Some funds have their largest holdings in the tech sector, some in banks and oil, some in the health sector. Wonder how much of that French lab company I own?

The unemployment rates, record number of people working including women and high school drop outs, bringing our boys home, restarting old factories, fair trade, etc….America is the best. Well, the economy was the crow jewel until the Wuhan virus came along. Trump peddles hope. Hope. Something that recovery programs peddle. Hope.

    lol. We are dealing with a genuine conspiracy nut here. Now that we’ve already established that Trump isn’t making any money at this…the next poorly reported story makes the rounds….
    So, some guy that Trump has played golf with twice since being President has a drug company and is going to sell a single burst of a generic drug that no one makes any money at ($5 per patient for the series? something like that). For all I know, he was specifically asked to. Does the drug work? Dunno, they’ll know soon enough. There’s probably a dozen other drugs that are worth a shot. At this point they should make a bunch of anything that might work, it isn’t like it’s a big investment.
    The important thing is that Trump doesn’t mention anything that actually makes a difference. The Blue Mob would rather die than use a Trump-approved substance.
    I don’t mind so much that our resident Troll gets all hot and bothered, but how the major newspapers report these things is unacceptable. The NYT articles on this sort of thing are filled with a lack of detail (is that possible?) in an attempt to push a political agenda. Xir Bertie can be excused for sounding like a Ron Lowe letter to the editor, but there’s no excuse for a major news institution to be this heinous.

    Heck, a thing just have to help 30% to be better than a ventilator. Probably less than that if long term health effects are factored in.
    I think that people like ventilators so much because you can count them. A thousand or five thousand is a number you can hold in your head, measure, think about how Trump didn’t design a better ventilator. It’s one of those cases (like most cases I sometimes think) where accuracy doesn’t buy you all that much understanding.
